Case Manager (Registered Nurse)


The Jamaica Country Coordinating Mechanism for HIV/AIDS has received a three (3) year (January 2016 – December 2018) grant from the Global Fund to fight AIDS, Tuberculosis and Malaria. This grant will be managed by the Ministry of Health, the Principal Recipient which has contracted the Jamaica AIDS Support for Life (JASL) as one of its implementing entities.  JASL and its sub-sub-recipients are seeking dedicated, dynamic, proactive and experienced individuals with strong team building skills to fill the following position: Case Manager

Chapter: Montego Bay, Ocho Rios & Kingston

Reports to: Regional Programme Manager

Summary of Duties

The Case Manager will have primary responsibility for the continuity of services and quality care specifically for persons infected and affected with HIV/AIDS. He/She will also be responsible for the support services programme and the coordination and follow-up of medical treatments/appointments.

 

Qualification

  •  Diploma in Nursing, Social Work or Social Sciences or relevant field.

 

Experience

  • Minimum of three (3) years’ experience working with MARPS population

 

Specific Deliverables

  1. Coordinate Positive Health & Dignity Prevention Workshops 
  2. Coordinate life skills sessions 
  3. Assist in training in the HIV/AIDS 101 workshop to volunteers and facilitators of Support Groups. 
  4. Ensure proper linkage to care, missed appointment and follow-up are carried out. 
  5. Increase retention rates among PLHIV on treatment 
  6. Follow through on referrals and/or calls made to JASL  
  7. Conduct phlebotomy  
  8. Take blood samples from persons requesting HIV test. 
  9. Coordinate Sessional Clinics with the Doctor and Nurse and schedule clients’ appointments. 
  10. Provide one-on-one, family and group counselling in the areas of managing personal hygiene and nutrition, identifying and taking immediate informed action to manage grief, death and dying and medical needs to PLWHA, their family members, peers, friends, etc. 
  11. Coach, counsel and advise family members of PLHIVA about needs, care, support and treatment. 
  12. Supervise Orphans and Vulnerable Children Program (Project Smiles) 
  13. Supervise collection and distribution of donations  
  14. Collect donated items, prepare and deliver Care Packages including food items, toiletries, medical supplies, equipment, clothing and disinfectant to clients. 
  15. Conduct HIV testing and counselling. 
  16. Conduct PIMA testing (CD4 count) and prepare monthly PIMA reports. 
  17. Manage and maintain the Ministry of Health Treatment Database and prepare monthly reports.  
  18. Submit samples to the Public Health Laboratory for HIV testing. 
  19. Collect results of samples submitted for HIV testing to the Public Health Laboratory. 
  20. Host counselling sessions for service users, volunteers, team members, people living with HIV/AIDS, family members of PLWHA, care givers and the general public. 
  21. Conduct primary interviews, evaluates needs and refers people living with HIV/AIDS, families and peers of PLWHA, clients, walk-in, service users, volunteers, care givers to general practitioner, psychologists, psychiatrists, CHARES, Brothers of the Poor, hospices, government health clinics etc. 
  22. Provide assistance in acquiring medication for service users who are living with HIV/AIDS by:

            i) Collecting the prescription from the service user;

              ii) Contacting the pharmacist;

     

              iii) Faxing or reading the prescription to the pharmacist; 

              iv) Serving medication as directed by the pharmacist; and

              v) Collecting, labelling and filling containers with the correct medicine.

  23. Makes regular visits to clients when hospitalized. 
  24. Assist family members, peers, friends, caregivers (in the event client dies) by providing information about the process involved when client dies such as: 
  25. Manages telephone, face-to-face and crisis counselling by: 
    •  Providing information to callers, walk-in clients and service users about the services provided by JASL; 
    • Facilitating counselling in the areas of coping skills, negotiation skills, empowerment, life skills, heightened awareness of HIV/AIDS, etc and 
    • Referring callers and walk-in clients and service users for medical care, emotional support and psychological welfare to the relevant professional, organizations and institutions
  26. Collaborate with other Agencies regarding HIV/AIDS/STI treatment and care.
  27. Attend weekly/monthly meetings such as Team Briefings, Briefings, etc.
  28. Represent JASL locally at Ministries, Funding Agencies, conferences, public events, speaking engagements at national and international AIDS/STI conferences, seminars and meetings with Affiliated Agencies. 
  29. Makes presentations locally on HIV/AIDS sensitization in hospitals, corporations, churches and various sectors. 
  30. Submits monthly and special reports on work completed and/or work in progress to the Regional Programme Manager. 
  31. Advises the Regional Programme Manager on matters impacting the Clinic/Organization. 
  32. Keep current about trends and changes relating to HIV/AIDS prevention, treatment and care. 
  33. Performs any other reasonable and relevant duties as assigned. 

Required Competencies

  • Proficiency in the use of relevant computer applications (MS Word, Excel and Power Point) 
  • Communication skills – Oral and Written, along with group facilitation and counselling skills, programme development and proposal writing. 
  • Able to work on own initiative with minimum supervision. 
  • Flexibility (including some Saturdays, Sundays and Public Holidays) and adaptability. 
  • Ability to develop, plan, and implement short and long-range goals.  
  • Ability to coordinate and implement multiple projects and events.  
  • Ability to handle high volumes of activity in a timely, professional and efficient manner. 
  • Skill in organizing resources and establishing priorities.  
  • Ability to use independent judgment and to manage and impart confidential information.  
  • Ability to work professionally and cooperate with team members and representatives of other agencies 
  • Strong time management skills, especially when facing tight deadline
